Student Health Insurance
California Health Sciences University College of Pharmacy requires all students to maintain an acceptable health insurance policy. Students must provide proof of insurance coverage each year throughout enrollment in the program.
If you already have health insurance: If you already have health insurance (employer-based for which you or a relative’s employer is paying) or other coverage, please provide proof to California Health Sciences University by completing the Student Health Waiver form and submitting it via email to the Office of the Registrar by June 30, 2017. Students enrolled in an HMO (out of state) should notify the HMO that they are enrolled at CHSU and verify out-of-network coverage.

If you do not already have health insurance: You may purchase Student Health Insurance through CHSU with our partner, Kaiser Permanente, which includes dental coverage administered by Delta Dental. Applications for the CHSU Kaiser Permanente Student Health Insurance will be available during open enrollment from June 1 – June 30, 2017. To enroll, please complete the Kaiser Insurance Enrollment Application and submit it via email to the Office of the Registrar by June 30, 2017.

Health Insurance Costs: An estimated yearly amount for student only health insurance is $3,140. The fee for this policy is paid directly to the school and will be billed on a bi-annual basis. A complete breakdown of Student Health Insurance costs is shown below:
Fall 2016 – 5 months of coverage $1,308.50 (Student only) $4,187.30 (Student + Spouse) $3,140.45 (Student + Child(ren)) $5,234.10 (Student + Family)
